I consent to the participant being in good health and in proper physical condition to play. I understand that the participant will engage in potential endeavors exemplified by, but not limited to, running, jumping, physical contact, etc. I understand and acknowledge that the participant may incur bodily injury while participating in the sports program, accordingly, I assume all risks inherited by the participant and accept full and complete responsibility for any injuries of any kind. I and the participant agree to adhere to the rules and regulations of the Peace360 Initiative Sports Program and be respectful to my fellow players and organizers.
